Uses, disclosures and authorization
The summary describes plan operations, communications and legally required or permitted disclosures. Hormel generally will not discuss health information with you or family without specific signed authorization. Basic eligibility / enrollment questions go to the third-party administrator. That administrator may share information relevant to care with an involved person unless you object, subject to limitations.
Before you decide
