When health information is required
- During new-hire enrollment, the book states guaranteed approval up to $350,000, if available up to that amount.
- After new-hire enrollment, adding or increasing coverage requires a Statement of Health and approval is not guaranteed.
- Previously approved coverage is retained when you request an increase.
- MetLife mails a health questionnaire; the election remains pending until a decision.
Cost
You pay for optional coverage. Your enrollment site provides the price; no premium table is included in the supplied book.
Make your election
- 1
Review offered coverage and cost in your enrollment materials.
- 2
Elect during the applicable enrollment window.
- 3
Complete any MetLife health questionnaire; do not treat a pending election as approved.
Next step
Check your new-hire approval window before electing optional life coverage.
