For you or your spouse, any new or larger amount needs a Statement of Health. MetLife mails you the questions, and the election stays pending until they approve or deny it.

When health information is required
- During new-hire enrollment, the book states guaranteed approval up to $350,000, if available up to that amount.
- After new-hire enrollment, adding or increasing coverage requires a Statement of Health and approval is not guaranteed.
- Previously approved coverage is retained when you request an increase.
- MetLife mails a health questionnaire; the election remains pending until a decision.
